* Says received senior default notice/payment blockage notice
* Says lender notified trustee of its debt holders that certain senior covenant
defaults existed
* Says letter indicated company was projecting defaults in its maximum fixed
charge coverage ratio
* Says letter indicated company failed to make June 30 interest payment
